The Springfield-Greene County Health Department is proud to offer a Community Health Advocate program. We know that a lack of access to healthcare and community resources can have a negative influence on health outcomes. Our Community Health Advocates work to address this risk by building strong relationships with individuals and families and connecting them to services in our community that can improve health outcomes.
There are two arms of our Community Health Advocate program:
If you or someone you know would like to speak to a Community Health Advocate about the services we offer, including connecting participants with partner resources like housing and transportation, fill out a referral here.
You will be asked to fill out a brief, confidential survey to determine your family’s needs, then meet with an Advocate to receive no-cost, personalized assistance.
